Changes to CD Requisition Forms

Please find below details of changes to the completion of CD Requisition Forms. We have been advised that it is a requirement for customers to complete the following fields in their own hand writing:

Purpose for which drugs are required - Section B

Please tick relevant boxes and add 'Other' information if required

Customer details - Section C

Individual Prescriber Code - Please enter your RCVS number
Individual Practitioners Name - Print name
Professional Qualifications/Occupation - Enter your qualification/title
Sign your name

Please note the required information is mandatory, we will not be able to process your order without all the correct information. Please note this change will take place from 1st February 2022.
